The review will use standard questionnaire and check list of report relevant to all thematic areas at review sites including Department of Health, Center of Disease Control, Hospital, and private immunization facilities at all levels (province, district and commune). The five type of standard questionnaire will be tested for each level prior to the review. Seven teams consisting from external and local counterpart will split to selected provinces to conduct interview in the field.
