- Use attached PSI quotation form No. PSI-SVC-20-07 Personal health and Accident Insurance 2020-2021
- Properly sign and stamp your quotation
- Indicate outside the envelop with: Quotation “Personal accident & Health insurance – No. SVC-20-07”
- Send your quotation via one of the followings:
- Hand-deliver it to PSI office at 11th Floor, Vinafor Building, 127 Lo Duc, Hanoi.
