2. Insurance package:

Detailed requirements for each level are in the proposal form attached.

• Number of staff level 4: 11
• Number of staff level 3: 14
• Number of staff/intern/volunteer level 2: 2
• Number of dependents level 1: 31

3. Proposals should include:

• Copy of business license
• Copy of similar contracts
• List of outstanding clients with contact points
• Signed and stamped quotation (use the attached form)

Interested suppliers please send the proposal in the sealed envelope to WCS Viet Nam office at D Building, No. 3 Thanh Cong, Ba Dinh, Ha Noi.
