HealthRight International is seeking a Nurse to join the Case Management team composed of a Nurse, a Social Worker and a Psychologist for the implementation of its Community – Based Care Options for Orphans and Vulnerable Children in Hanoi.

ORGANIZATIONAL OVERVIEW
HealthRight International (HealthRight) is an international health and human rights organization. Reaching out to the most vulnerable and marginalized populations, HealthRight works with local partners to build sustainable access to health care for disadvantaged populations. 
HealthRight is seeking a Nurse/Physician Assistant to join an interdisciplinary Case Management Team for the implementation of the project: “Promoting Community-based Care for Orphans and Vulnerable Children (PCC-OVC)” in Hanoi.

Responsibilities and Duties
- Establish and maintain relationships with state and non-governmental organizations to build support network for OVC and their families
- Together with the interdisciplinary team develop and fulfil  a case management plan to provide social, medical and psychological support to children and family members
- Asses health care needs of OVC and their caregivers and provide appropriate referrals to medical facilities
- Provide basic health care and first aid care
- Conduct home visits to provide information and counselling regarding ARV treatment and adherence, special needs of HIV positive mothers and/or her child, nutrition, breastfeeding and regular medical monitoring for children
- Ensure that nursing supplies (disinfectants, bandages, etc.) are monitored and reported on time
- Provide side-by-side mentoring and on-the-job training during joint home visits with the HBC workers
- Conduct with the case management team monthly case conferences for HBC workers
- Participate in project monitoring and evaluation and results dissemination activities

Administration
- Keep information on mothers, their children and families confidential
- Collect data on provided health care support; report data to Program Coordinator on a regular basis
- Participate in all necessary organizational meetings and conferences

Requirements
- Degree in Nursing/Physician Assistant
- At least 3 years experience in health care provision field
- Prior work with children, particularly children living with or affected by HIV
- Skills working with most at-risk populations
- Skills in working with children and families
- Fluency in Vietnamese and proficient in English
- Highly developed interpersonal skills
- Proficiency in Microsoft Suite including Excel and Word
- Ability to maintain professional case management record
- Ability to maintain discretion and confidentiality regarding work related issues
- Ability to travel within the suburb areas of Hanoi
