Research Agencies/Consultants Baseline/Endline Survey and Operational Research for School Eye Health Projects

The Fred Hollows Foundation (FHF) is a development organization working in more than 25 countries. We focused on strengthening eye health systems and the treatment and prevention of avoidable blindness.

We are looking for qualified research agencies/consultants to  conduct a set of surveys and researches (called school eye health research project – SEH) to support 2 projects including the Vietnam Child Eye Care (VNCEC) (2016-2018) and Better Eyes for Better Education (BEBE) (2016-2019) projects focusing on school eye care in Vietnam as TOR attached.

Research agencies/consultants are interested in the study, please visit our website www.hollows.org.vn for more information. All following documents response to this RFP will be sent to FHF by 2 deadlines:

1. The project proposals (using the template provided in Appendix 2) which describe study design (for each research activities). Please be noted that there will be 2 separated proposals for baseline/endline survey and for operational research which will be submitted in different deadlines to facilitate for high quality proposals.

2. A cover letter addressing the key selection criteria and including two references

3. Curriculum vitae for individual consultants or the Principal Investigators. If applying as a team, please also provide a team capacity statement which describes the team, collective experience and track record of working collaboratively with two examples of previous written work (e.g. published reports)

4. A detailed budget that must include detailed costing for all study costs including but not limited to:  personnel, study design costs, training costs (materials, refreshment, s1per diems, etc.), field materials, field salaries, field transport, data entry personnel and equipment, and administration costs. It is requested to submit the proposed budget in Vietnam dong (Appendix 3) and separated for each proposal.

Responses to this RFP are due no later than at 4:00 pm on 26 September 2016 for baseline/endline survey proposal and 5 October 2016 for operational research proposal.  Proposals delivered and/or received after this deadline will not be considered and will be disqualified. All the above documents must be in sealed and separate envelopes. Please name as “SEH_Baseline/Endline survey proposal” or “SEH Operational research proposal” and specify technical vs financial proposal on the envelope and then send it to:
